As soon as an injury is reported or discovered, begin the filing process.  Employees have only 30 days after the accident to report a claim.  Employees must provide notice to the Employer of the time and place where the accident occurred and the nature of the injury.

2.      Be Thorough and Accurate: Complete every section of the form honestly and with as much detail as possible.

3.      Keep Records: Save copies of all reports and correspondence for your records.
